Description Integration of different thematics (social care, prevention, health). Integration of differents stakeholders : national operators, local authorities, private and public service providers (hospitals, clinics, health praticians…) Integration of different admin levels : region, departments, local authorities, through territorial plans and local health agreements.

> support loss of autonomy elder persons, handicaped persons, and persons having specific difficulties
Short Summary This scheme aims to support the evolution of social care services and institutions to enhance quality of life for elder people, handicaped persons, and persons having specific difficulties.

Its principles are to propose :
> a various offer, innovative and sticking to local needs and out-of-institution answers,
> make the offer coordinated and accessible
> allow a territorial equality for proximity care as for more specialized
> propose an efficient offer, consolidating and optimising the existing structures

This goal suppose to work on social care efficiency assessment, better human resources management, and more cooperation and integration between thematics and structures. The Regional Health Authority can use different agreement protocols with structures of the social care field, and project calls.
